Community Resources / Nonprofit Organizations

Area Agency on Aging (AAA)
For aging/disabled individuals, AAA serves as an access point for information and connection to community-based programs and services. If you live in the metro Atlanta region, your designated AAA is Empowerline. For all other Georgia counties, you can find your AAA at https://aging.georgia.gov/locations.

Adult day respite program for individuals with dementia. This program operates three days a week from 10 a.m.- 2 p.m. (Call RCA for info on how to apply)

Providing: Geriatric Social Work, Dementia Support Program , Educational workshops , Support groups for caregivers
Offer: 3 in 1 bedside commode, Canes, Hospital bed, Hoyer lift, Shower chair/tub transfer bench, Walkers and Rollators, Wheelchairs and Scooters
For wheelchair orders, patients are required to go through a seating clinic. This includes a physical therapy evaluation, technology therapist evaluation, and a face to face appt with our Nurse Practitioner. Please contact your case manager for wheelchair orders or further information regarding this process.
Not all DME is covered by Medicare. Friends of Disabled Adults and Children (FODAC) is a nonprofit organization that sells refurbished medical equipment. FODAC can be a great resource for accessing equipment at a lower cost.

At Home Care

At home care offers nursing care, personal care assistance, and companion/sitter services. These services include assistance with activities of daily living (bathing, toileting, grooming, dressing, etc.), nursing services, transportation, and help with household tasks. At home care is an out-of-pocket cost. The only at home care that is typically covered by insurance is skilled nursing care, which requires a doctor’s order. For questions about coverage, please contact your case manager.

Long-term Care

Assisted Living Facilities (ALF)

Assisted living communities provide services including medication management, meals, and assistance with ADLs like grooming, bathing, and toileting. Assisted living is private pay. For lower income individuals, you can contact your local Area Agency on Aging for information on Medicaid programs that can provide financial assistance for assisted living.

Memory Care

Some ALF have designated memory care units. There are also facilities that are entirely memory care communities. In a memory care environment, the same basic services as assisted living are provided. However, memory care offers specialized care to seniors with memory loss, Alzheimer’s, and other forms of dementia. Like assisted living, memory care is private pay. Family Medical Leave Act

Family members caring for a loved one may qualify for up to 12 weeks of FMLA, which is time off from work to assist with transportation and provide help with activities of daily living. Please contact your case manager for more information or assistance with applying for FMLA through your employer.

Nursing Homes

Nursing homes offer a higher level of care than assisted living by providing 24-hour nursing care to individuals with severe cognitive or physical conditions. Nursing homes may also offer rehabilitative services, including occupational, physical, speech and respiratory therapy. Nursing homes generally accept Medicaid or private pay. To check eligibility requirements or begin the application for nursing home Medicaid, you can contact your county DFCS office.
